私が注文した時の画面ですが、何故か送料が$13.50になってますね。
そこで隣にある"learn more"をクリックしてみると。

・送料は商品価格の45%。最小$40。カタログに表示のある大型アイテムは別料金
・各種税金（関税、消費税等）は別
・注文受付から到着まで7-10営業日
・在庫有り商品の多くは48時間以内に発送（土日祝日を除く）
・大きいもの、かさばるものはさらに送料がかかることがある。送料が$10.00を超える場合は事前に知らせる
・支払いはAmex、Master、Visa
・パッケージが複数になることがある。配達も複数回になることがある
・注文を送信したら、上記内容に同意したものとする

結局、表示されている送料には意味が無いようです。

さらに、質問に答える必要があります。

回答は私の場合です。
問題は2番目の質問ですね。
別送になった場合の送料がわからないので、一応Noにしておきました。
個別に送料がかかったりすると大変そうなので。

今日注文した際には、「Required Questions:」の項目はありませんでした

クレジットカード番号を入力します。
CVNというのは、カード裏面の署名欄に印刷されている番号の下3桁。
『Submit Order』で、一応注文は完了です。
注文完了画面は保存するなり印刷するなりしておきましょう。
私は印刷プレビューからpdfに書き出して保存しています。
